Please enter all relevant information, including the name, street address, and municipality, along with the county and township. This form works well with smart devices for taking photos and submitting them at the same time. Maximum five photos per submission.

Please re-name your photo files with a meaningful name if possible: such as Glencoe_Knox_1.jpg; Glencoe_Knox_2.jpg for example.

Please add additional information in the comments section, such as whether the location has been re-named, destroyed or decommissioned, construction dates, whether re-built or re-located, and any other information which may be of use or interest.
